Mutation Driving Pediatric Brain Tumors Identified


A lethal brain tumor in children has been found to contain an unusual mutation that may help to classify, diagnose and guide the treatment of the tumors, say scientists.

The researchers led a study of pediatric low-grade gliomas, samples of which were collected through an international consortium organized by brain tumor specialists at Dana-Farber/Children's Hospital Cancer Center. Their findings are being published online by the Proceedings of the National Academy of Sciences (PNAS) the week of April 29.

Low-grade gliomas are the most common type of pediatric brain tumors, diagnosed in about 1,000 young patients annually in the United States. There are about 30 distinct types of these tumors, which arise from specialized cells called glia in the brain. Low-grade gliomas are generally slow-growing, said Keith Ligon, MD, PhD, a senior author of the study, but they behave unpredictably and can be life-threatening.

The investigators focused on diffuse low-grade gliomas, so-called because they lack a tumor mass but spread throughout the brain. As a result, diffuse gliomas often recur after surgery and are more likely to evolve into lethal glioblastomas than are non-diffuse low-grade tumors. "Many of these patients do well, but it's hard to generalize as the tumors are difficult to diagnose and study because without better tools pathologists can't name them consistently," explained Ligon, who in addition to being a researcher is also a neuropathologist. The research was undertaken in hopes of identifying a common genetic alteration that could be used to better define and design treatments for them.

The researchers analyzed DNA from 45 tissue samples collected from seven institutions in collaboration with Rameen Beroukhim, MD, PhD, a Dana-Farber genome biologist and co- senior author of the study. They looked for mutations caused by extra or missing copies of DNA code in the tumor genomes.

One alteration stood out: a gene called MYBL1, a transcription factor important for controlling other genes, was rearranged and missing a part of its genetic message in nearly 30 percent of the diffuse tumors categorized as grade 2 in terms of aggressiveness. The scientists went on to show that the mutated version of MYBL1 can cause tumors in mice. Previously MYLB1 was not known to cause cancer, but a closely related gene, MYB, is one of the oldest "proto-oncogenes" - a normal gene that can become a cancer-causing gene.

"The creation of these truncated genes, reminiscent in structure of the viral oncogene, is a potential driver for this type of tumor," said Lori Ramkissoon, PhD, co-first author along with Peleg Horowitz, MD, PhD, a neurosurgery resident, both of Dana-Farber. "It gives us something to follow up on and investigate the function of this gene. It may lead to a specific test for diagnosing these tumors, and we will also try to determine whether patients who have this mutation do better or worse than those lacking the mutation."

Foods and Drugs that Don't Mix


Foods you should not mix with your prescribed medications.

We all know not to mix drugs without consulting our doctor, but how many of us know that some foods don’t mix with the drugs we take? Here are some examples of foods never to eat with:

Grapefruit:

Cholesterol: Lowering statins like, atorvastatin (Lipitor)

Calcium channel blockers

The heart-beat regulating drug amiodarone (Cordarone)

Immunosupressants, including cyclosporine (Sandimmune, Neoral) and tacrolimus (Prograft)

Antiretroviral agents like: Saquinavir (Fortovase)

Grapefruit can heighten the effects of the medication even days before taking the medication, so you should never eat grapefruit when taking these medications.

Green Vegetables:

The blood thinner: Warfarin (Coumadin)

Green vegetables can weaken the medication leading to dangerous blood clots. Your doctor will regulate your vegetable intake when prescribing this medication, so if you haven’t been eating vegetables, do not start.

Dairy Products:

Quinlone antibiotics such as Ciprofloxacin (Cipro)

Tetracline antibiotics such as tetracycline (Achromycin), minocycline (Minocin) or doxycycline (Vibramycin)

Never consume dairy when taking these medication because calcium in the dairy products binds with certain antibiotics. You must wait at least two hours after the medication to use dairy.

Caffeinated Beverages:

Asthma drugs (Ventolin, Provetil or Combivent) or epinephrine (Primatene Mist)

Quinoline antibiotics (”see dairy products”)

Anti-anxiety drugs such as diazepam (Valium), alprazolam (Xanax) orlorazepam (Ativan)

Histamine blockers, including: Cimetidine (Tagamet), famotidine (Peptide) or ranitidine (Zantac)

The side effects of the medication with caffeine will make you wired and hyper.

Alcohol:

Codeine based pain killers combined with acetaminophen (Perocet, Roxicet), meperidine (Demerol) or hydrocodone with acetaminophen (Vicidon, Lorcet)

The asthma medication theophylinne (Slo-bid, Theo-Dur, Uniphyl)

Beta-blockers propanolol (Inderal), isosorbide dinitrate (Isordil, Sorbitrate) Or nitroglycerin

Statins (see “grapefruit juice”)

The antibiotic metronidazole (Flagyl)

The anti-depressant bupropion (Welbutrin) and anti-anxiety drugs( see”Caffeinated beverages”)

Never drink alcohol when taking these drugs. You could go to sleep which is extremely dangerous if you are driving. It could drastically slow you heart and your breathing rate. It could also cause seizures and lower your blood pressure to a dangerous level.

There are other foods you should not mix with certain medications. Always ask your doctor and pharmacist about any food to avoid when taking medications.

The History of Tooth Brushing


About a year ago, I heard an NPR story which erroneously said that tooth brushing was only about 100 years old. Sooooooo wrong. Keeping your teeth clean is not a modern concept, though the advent of tooth paste as we know it is. In actual fact, brushing ones teeth and good oral hygiene has been around for Millennia. Excavations from all over the globe have included little sticks, some chewed to make fibrous and some with bristles, just for the purpose of tooth brushing.

Ancient Egyptians have been notorious for having bad teeth. Sand got in everything, from the bread to the beer, and wore down enamel with the simple act of chewing. Bad breath was treated with honey, of all things. But you know what they did do? Brush teeth. They chewed on the ends of some fibrous plants to make them ‘bristly’ and brushed away. There have been some indications that they may have even used stale urine to brush teeth! Sure it had ammonia, but yikes…

Ancient Rome had a regimented feel to it. Every aspect of life was governed by the right way of doing things. The great thing about Romans is that they recognized ‘the right way’ in the peoples they conquered and Romans were known teeth brushers, just like the Egyptians. They were great historians and record keepers and even the most mundane of details were written down for the good of the Empire. Historical records show that primitive tooth picks were chewed on and used to clean teeth.

Ancient China had their own version of the chewed branch, but in the middle ages they developed a tooth brush with bristles similar to those we use today. Stiff hog hair was apparently great for this, and a variety of things were used in lieu of the modern ‘paste’.

Even in the darkest part of the Dark Ages the people of Europe brushed their teeth. Yep, even in England, and there is ample historical evidence to prove it, not only in archaeological example, but in historical record in the form of recipes. Tooth brushing was considered healthful and hygienic even in times of war. Napoleon Bonaparte regularly used sand powder and opium to brush his teeth with a sliver gilded horse hair brush. Yeah, yuck, but at the time that was the height of oral care. The London Museum of Science has the brush (well, the alleged brush. His name isn’t on it or anything) and you can view a picture at 

The first toothbrushes were mass produced in England in the 1780’s, but it has been around for awhile.

That brings us to the good old U.S. and colonization. A boar’s bristle brush would have been used and perhaps some baking soda after it was available widely and mint leaves have been known to have been chewed for fresh breath. The first patent for a tooth brush in the U.S. and the first mass-production of a tooth-brush both occurred in the 1880s, but things really didn’t get going until the 20th century with the advent of nylon bristles and the first commercial toothpastes as we know them today.
